The Program Coordinator will lead the daily operations of a community-based crisis and transitional housing program located in East Patchogue. This role is pivotal in ensuring that all services delivered support recovery, safety, empowerment, and the overall well-being of individuals facing mental health challenges. The Program Coordinator will oversee staff supervision, service delivery, compliance monitoring, and program development to foster a trauma-informed, client-centered environment that promotes stability and growth.

Key Responsibilities:

  • Oversee and coordinate direct care and short-term crisis respite services provided by program staff.
  • Conduct weekly one-on-one and team supervisory meetings, including clinical oversight and documentation review.
  • Deliver ongoing training and support to team members to ensure high-quality service delivery.
  • Perform intake interviews, screenings, and assessments for potential program participants.
  • Support efforts to maintain full program occupancy by managing the intake, documentation, and admission processes.
  • Review and monitor client records and documentation for accuracy, completeness, and timely submission.
  • Provide in-person and on-call crisis intervention support as needed.
  • Ensure program adherence to regulatory standards set by NYS Office of Mental Health, LIHTC, and other oversight bodies.
  • Collaborate with property management to maintain facility safety, cleanliness, and functionality.
  • Serve as a liaison with community organizations, health providers, and other external stakeholders.
  • Guide staff and residents through critical situations, making appropriate community referrals and connections.
  • Assist with coordinating staff schedules, including per diem and relief staff.
  • Participate in hiring, performance evaluations, and, when necessary, disciplinary actions.
  • Step in to provide direct services in times of staff shortages or turnover.
  • Adapt work hours to meet the needs of the program, including occasional evenings or weekends.
  • Uphold and enforce agency policies and procedures at all times.
  • Stay up to date on all required certifications, training, and agency compliance mandates.
  • Perform other duties as assigned by the Program Director.

Required Qualifications:

  • Master’s degree in Social Work, Mental Health Counseling, Rehabilitation Counseling, or a closely related field.
  • Professional licensure (e.g., LMSW, LCSW, LMHC, CRC) preferred.
  • Minimum of three (3) years of experience working in a mental health or supportive housing setting.
  • Demonstrated leadership abilities, including experience supervising multidisciplinary teams.
  • Strong verbal and written communication skills.
  • Ability to assess complex situations, make sound decisions under pressure, and maintain client dignity.
  • Experience working within a trauma-informed, recovery-oriented care model is highly desirable.

Supervisory Scope:

This position provides direct supervision to clinical and support staff, which may include Counselors, Service Coordinators, Behavioral Health Specialists, Recovery Specialists, and interns as designated by the Program Director.
